There are 4 different buildings on our Waldron campus in Hutchinson. We also have facilities in Hutchinson at 1100 North Main and in South Hutchinson. We can help you find the correct building for your appointment.

  • New Patient Form *This form is not used by our ENT department. Forms for ENT are sent by mail prior to your appointment.

  • One-Time Authorization Form

  • Photo ID

  • Insurance Card

  • All current medications in original bottles

  • Previous Medical Records (if possible)

WHAT TO EXPECT AT YOUR FIRST APPOINTMENT 

To register as a new patient we will need your:

  • Name

  • Address & Phone Number

  • Date of Birth

  • Insurance Card

The Hutchinson Clinic contracts with most insurance companies including Medicare and Medicaid and Blue Cross Blue Shield.

Many insurance plans require a co payment for each visit. We ask that you please make this payment before each appointment. This can be done at any reception desk or the Business Office located on the lower level of the 2101 building.

For your first visit it is helpful to arrive early to check in. If you need to cancel your appointment please call us 24 hours in advance to reschedule.
